At Home Support is an independent not-for-profit subsidiary of Hospice of Michigan which offers a continuum of proven advanced illness management solutions for patients facing serious chronic illnesses. At Home Support’s clinicians are experts in providing patients with pain and symptom management related to a serious illness; helping patients and caregivers navigate through the healthcare system; assisting patients and caregivers through difficult and complex treatment choices; and providing emotional support to patients and their families.
Advanced Illness Management refers to home-based care programs established to ease the transition between curative and comfort care for seriously ill patients who lack coordinated hospital, home health, and hospice care.
At Home Support provides specialized supportive care for seriously ill patients dealing with chronic illnesses. At Home Support’s services focus on providing patients with relief from the symptoms, pain, and stress of a serious illness. The goal is to improve quality of life for both the patient and their families. Working in conjunction with a patient’s primary physician, At Home Support’s clinicians manage symptoms such as pain, shortness of breath, fatigue, constipation, nausea, loss of appetite, difficulty sleeping and depression. At Home Support’s services help patients gain the strength they need to carry on with daily life, navigate the healthcare system, and make difficult and complex treatment choices. At Home Support’s also provides patients and their families with the emotional support that is critical in these challenging situations.

At Home Support treats patients suffering from serious chronic illnesses such as cancer, congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD), advanced dementia, and amyotrophic lateral sclerosis (ALS). At Home Support’s services are most appropriate for patients with advanced stages of these diseases (e.g., Stage IV cancer) and/or patients who have earlier stages of these diseases but also have frequent visits to the hospital.
At Home Support’s clinicians work in conjunction with a patient’s primary care and specialist physicians. Our clinicians provide the physician with a copy of the patient’s plan of care and communicate with the physician on a regular basis to discuss any recommendations for changes to a patient’s care plan.
At Home Support’s services are provided through an interdisciplinary team of RN case managers, master social workers, patient family assistants and a set of core volunteers, all of whom specialize in caring for seriously ill patients dealing with a chronic illness. The At Home Support clinical team works together with the patient, their families and their physicians to ensure that our services are tailored to the specific needs of each patient and family.
All patients receive services from the At Home Support interdisciplinary team within the comfort of their home, in addition to 24/7 tele-support service.
At Home Support provides 24/7 support – including at-home visits from an RN case manager in the event of an urgent situation or crisis as well as 24/7 telephone support.
At Home Support services are not covered by most health insurance plans, however the service is offered in conjunction with most health insurance plans and accountable care organizations.
