Our RN Case Managers develop strong relationships with the patient’s primary physician and ensure the patient’s plan of care is reviewed and updated with any changes as requested. If needed, we will regularly call our patients physician to ensure that he or she is always up-to-date on every patient’s status. We serve as the patient’s primary contact and try to anticipate problems before they occur and work with the patient and their family to prevent a crises situation. However, when a crises does occur, the patient can call our tele-support team 24/7 so that we can respond immediately whether it occurs after hours, over the weekend or in the middle of the night.
